While they don’t result in hearing issues, prominent ears can have significant psychological impact on children. Most kids do not sense a problem until about age 6, when they start to become more aware of their appearance. Teasing by other children can result in reduced self-esteem. Many adults who decide on surgery report the damaging effects of relentless taunting they experienced as kids.

The most common cause of prominent ears is failure of the upper fold of the ear to form completely. This results in protrusion of the upper portion of the ear. Over-growth of the bowl of the ear may occur as well.   Otoplasty, sometimes called ear pinning, is the procedure used to treat prominent ears.  It is generally an uncomplicated cosmetic procedure performed to make prominent ears less prominent. It’s important to note that procedures performed solely for cosmetic purposes are typically not covered by insurance, though some amount of coverage may be available. Different types of otoplasty can also be performed on very large ears, ears damaged by trauma, or ears affected by congenital deformities.

How old should a child be at the time of surgery? Ears have generally grown and matured enough by age 6 to undergo the surgery. Since it is at around this age that children may become sensitive to their appearance, this is a good time to consider the procedure. It is very important, however, to allow the child to help make the decision for surgery. They are old enough at that age to have an opinion, and getting their input is an important part of the treatment process. Adults are also candidates for otoplasty.

During the surgery, general anesthesia is used for children, while adults can usually undergo the surgery with local anesthesia, with or without sedation. An incision is made behind the ear, and the cartilage – the strong, flexible material that forms the ear – is re-modeled to its new shape. A few sutures are used until the tissue heals, and the scar is hidden behind the ear. The surgery will generally take two to three hours. Every case is different and individualized care is always provided to every patient.

Since otoplasty is performed as an outpatient procedure, patients go home soon after the surgery is completed. The soft, bulky dressing placed after the surgery will be removed the following day, and a lighter dressing will be placed. After about a week, that dressing will be replaced by an elastic headband to be worn constantly for a week, and then at night for several more weeks. Complications are rare, and patients can return to a normal routine in about a week.  As with any surgical procedure, every patient is different, and Dr. Bridges will discuss your expectations with you before surgery.

The first ever Commonwealth Facial Plastic Surgery “Evening of Beauty”, held on March 10, 2011 was a huge success. Almost 90 people attended, in the middle of a driving rain! One ongoing “hot topic” in facial aging is volume loss. This is certainly not a new concept, but more and more focus is being directed toward restoring facial volume.  Think about what a youthful face looks like.  A baby has full, rounded cheeks.  The overall shape of the face is an upside-down triangle.  As we age, fat loss occurs under the cheek bones and in the temples, and lower cheek fat sags somewhat creating jowls.  The face takes on a more square shape. As jowls get worse and midface and temple fat shrinks, the face becomes shaped more like a triangle again, but this time the triangle is “right side up”. Options for correcting this loss of midface volume include implants, midface lifts, and fat injections, to name a few.  There is really no good surgical treatment for temple wasting, however.  Sculptra is a new injectable that is really a new concept in “fillers“.  It is composed of a material that has been used in absorbable sutures for years.  When injected beneath the skin, Sculptra causes your body to produce collagen. The filling effect of Sculptra is not from the material injected, but from your own collagen.  Sculptra requires usually 3-4 injection sessions, spaced about 6 weeks apart.  The results are very gradual, but long-lasting.  Expect Sculptra effects to last up to 2 years!
